VACCINE SURVEY IS NOT REQUIRED
The vaccine survey request is NOT REQUIRED. That terminology is used only to persuade you to comply. Again, IT IS NOT REQUIRED. Having a vaccine is not a requirement of your job! You DO NOT have to answer the survey.
