Yes, I know several people that have however they took the form to apply for one off of teamworks recently with no context. Yet another violation of ADA law.

@f3 You need a letter from your doctor explaining why you cannot go to the office. In my case, the letter had an expected date to go back, and the doctor reviewed it twice. The HR will also call you to confirm the information in the letter before approval.

@tg Not everyone in accommodations got laid off, some pending cases are getting reassigned prior to Jan 1.

They are outsourcing to MetLife starting January 1 so stay tuned. They laid off all the people in HR who were doing the requests.

Of course, you had to find out other ways like from a disabled person in HR because it wasn't announced.

Yes, engage employee relations for an accommodation request. Apparently it varies with who you get on the timing for recert 6 mos/1year but there is a major push to try to accommodate every situation within an office where they have a solution. Make sure your documentation covers everything on your disability and accommodation needs.

Probably not. Work is performed in the office. Covid is over. If you are not customer facing, get back asap while you can. Anyone in the Philippines or India is readying to take half your pay for a chance to take it. Happening quickly.
