just tell us what happened. the answer your question is standard HR process which you can google. what do you need to say? collecting info on a standardized process is boring
I had something happen on my team but in order for an investigation to take place, you have to go on record as well as the person being accused will be interviewed. In our case, we couldn’t be anonymous.
