I feel like HR is making it more complicated to get my accommodation approved this time around by not responding and claiming they're not receiving paperwork. Anyone else having that problem? Is there an appeals process if it gets denied? Or can I have a different HR rep review it and explain my issues with a particular rep?

It’s a deliberate move on their part. At this point, you’ve become deadweight. They are ready to replace you. I’m in the same position as you except they are telling me it hasn’t been approved BUT it hasn’t been denied. I’m waiting for the Bank to make one more wrong move and I’m pushing that attorney button. I already have two lined up ready to file a complaint on my behalf. Keep detailed documentation. You’re going to need it.
