One time I was given an 'anonymous' survey about job satisfaction at work, and wrote in one comment 'I don't think its fair to tell employees their responses are anonymous when they're not' and I was called into a meeting with HR where they assured me my results were 100% anonymous. They even showed me a printed out version with my name and comment on it.

Years ago I wrote a comment, professional, polite, but critical of executive leadership. Within a week of completing thar survey I got call by my boss and was asked if I wrote a particular comment. I told her I thought the survey was anonymous. She said it is anonymous. Then why are you asking me about this comment, how do you know it's mine. It's your style of writing. I laughed and said I'm not going to confirm or deny any comments I've may have provided in a survey. These surveys are anything but anonymous and a subsequent manager later affirmed they are NOT completely anonymous. I never again answered another survey truthfully.
