I don't think it was human error at all. It was the refinery itself or birds or something.
Just goes to show. These decisions have real consequences.
You offshore work for a fraction of the cost and you can distract the whole org with a very long reorg.
Those actions will supposedly benefit shareholders in the short term, but they also deteriorate our operations, expertise, and culture, which lead to these types of issues.

They literally provided the data showing that reorgs cause more safety incidents then proceeded to execute one of the most chaotic reorgs imaginable.
I was there in ES when management decided to drop the OE moment from meetings in favor of a D&I moment. Just another stupid move by leadership
We need more OE moments before meetings to stop the safety events.
Maybe we should ALL have a safety standown until this BS and complete lack of qualified leadership is addressed.

I thought the same thing . The way this reorg happened was based on favoritism and not qualifications. A lot of good people were left standing and a lot of people that are friends with leaders stayed. New org Oct 1st , explosion Oct 2nd. Hopefully we don’t have more incidents , but it is concerning for the safety of our operations folk.
