Can someone please post the details of the severance package? Is it two weeks pay for each year of service or something else? What's the max you can get? And can you get it as a lump-sum?

My package I received today clearly says paid out in lump sum. Copy paste straight from the doc:
Severance Eligibility:
Severance is paid in a lump-sum payment under the terms of your severance plan. To be eligible for severance benefits, Windstream requires that you sign a Waiver and Release Agreement.
Just an FYI, the package is a continual pay package, NOT a lump sum. You get paid every 2 weeks until your severance is paid out. Max amt of pay will be 6 checks totaling 12 weeks (Max allowance).
Right you can have been there for 11 years as I was, and max out at 12 weeks...
it is a lump sum*

I think it may vary based on department or where you work. My offer was different than stated above....
Two weeks pay for each year not to exceed 3 months pay.
