Before you make that decision on taking the extra months of COBRA or the cash, be sure you understand the tax implications. You may end up giving back 40% of that cash in taxes
email transition.support at intel.com and tell them you haven't received it. I believe that you have something like 45 days after the termination date to sign up for cobra. That is a US law, not an intel decision. there will be no gap in coverage is you sign up for cobra.

For the conspiracy minded folks, the whole reason we have all this stuff Exhibit A and all that mumbo jumbo is that Intel wants to avoid any lawsuit. A lawsuit by any individual will cost them more than your separation package.. It is in Intel's very best interest to honor everything they promised.
Mine says 30 days from receipt. But of course it didn't come registered mail, so they have know way of proving you ever received it...
And think hard and do the math before you sign for the 12 month cobra. For me and my family I can take the $9k, and pay the extra 6 months of cobra myself, and come out ahead.
Got mine Tues 5/26. After calling Friday before, they said I should have received it. I noticed signing for the 12 mths cobra has to be turned in by May 31st. They should have these to us 2 weeks in advance, but as always they slip on delivery.
45 days after receipt. Once you sign it, it becomes effective 8 days after Intel receives it.
