Unfortunately, I’m having health issues and it’s seems likely I’ll need to take a long term leave, and perhaps long term disability. With the coming reorg, I find myself wondering if it’s time to push up my timeline. Worst case for me would be being let go, and losing my long term disability insurance. Figured I’d try the give mind here, and perhaps get a lurking HR rep to give honest advice. I’d hoped to delay this until I couldn’t, but now I’m thinking I should be proactive to preserve my disability insurance…

Health comes first - take care of yourself and do what's needed to come back stronger!
There was a famous story about a guy at Enron who went on leave and filled up his voicemail message box so HR couldn't even leave a message that he was fired. They ended up having to fire another guy to meet their targets so he was spared (for a while).
Do it now. FMLA can protect you for a while. Good luck dealing with the process, nobody knows what they are doing and it is basically a full time job for you to figure it out for them. Seriously. Nobody knows anything, be ready for that.
Being on leave could actually work in your favor as HR would be reluctant to let you go while you are off as it could be seen as discrimination due to health issues. I would go on leave as soon as possible and stay on leave until the layoffs are long completed. And don't answer any Chevron phone calls or emails while on leave!
Take care of yourself first.
If you are having health issues which will lead to Leave followed by Long Term Disability you should take care of your health first. You have had employment for benefits you earned for long term health issues. With ROM there is no guarantee you will have a job. With all the chaos with HQ move and ENGINE added into the mix the timeline may be unpredictable. Putting your health aside for uncertainty makes no sense. You should have until at least early 2025 to sort out the details. Good luck to you.
Best wishes, OP. I hope you will get better. Take care of yourself.
Take your leave. You matter more.
I would run to the doctor if I was you.
