The organisation has sleepwalked into doing things the most complicated way possible. And now it wonders why it can't make any money. It spends more time and thus money on paperwork and process than actually doing. The clients are sick of it. The paperwork is out of all proportion to the tasks being done. Yes you need PM's and TA's when you're doing a big project. But when you're just updating where a log file is saved - come on! They talked the talk on streamlining, but never followed through.
