Why CVS PBM lost its biggest health plan client, 40 B dollars in revenue

I know they will not punish real culprits behind loosing biggest health plan client of CVS PBM, one which had 40 Billion dollars revenue.

Their Client Services/Operations department dropped the ball. CVS ELT made their president a skapegote and let him go. Real culprits are still there. No wonder, CVS Caremark may loose another big client due to bad client services and operations.
