Are these normally delivered with a formal letter from hr with a stated action by date, or just a written action plan from your manger?

@a1 Oh, I know people on PIP who got laid off. It doesn't offer any protection.
@cy If you are a good employee there is no need to put you in PIP in the first place.
PIPs always involve HR - and many times it is somewhat futile and impossible to meet the criteria. However, if you are actually a good employee, they will work with you to survive it.
I was given PIP this January. It's just a letter from HR and my manager went through it with me. It says there's no timeline on it. Then I know I'll be in the next layoff list whenever that happens.
PIP are rarely used nowadays as in a counterintuitive way they offer some protection against layoff (as in theory you should be allowed to complete it before being fired).
