Asking for a break

I need time off for my mental health. How do I even bring this up to my manager without it sounding weird or hurting me later?

Reach out to EAP (spring health) I did a mental health leave last year. Stress was too much. They can provide the answer to your question. Best of luck I totally understand!

Probably worst time in history to be complaining about human issues like mental health. Your job won't exist much longer, enjoy having somewhere to work while you can.

Location dependent, but you can sign up for an occupational health assessment, which your manager doesn't need to know about or approve.
You'll be assessed by an external OH team, who will then make a recommendation to your manager based on their assessment.
They may ask for details and proof of your condition from your Dr or local health service.

Once you have their recommendation (Which may be to give you time off, or a flexible working arrangement), your manager has to abide by it.

However if you're not willing to go through the OH process, or if you don't have a recognized condition, your options are extremely limited.

Focus on your hobbies and look for a job elsewhere. Travel, take time off.

Use FMLA (federal law) for mental health, skip your manager. You will hurt yourself, though (no promotions for a while, etc), but your mental health is more important. The company cannot do anything during the FMLA period (90 days?).
