Help needed for short term or long term info

What documents do I need to apply for short-term disability? I recently fell down the stairs, and my doctor advised that I need at least two months of rest.

Also, is there a specific place I should apply or a phone number I can call for assistance? I would appreciate your help. Thank you.

I'd say next time, shoot for 2 flights of stairs. A necessary mental health break first and foremost, and physical healing will be secondary. =)

@OP Look under benefits in system and scroll down to review Short Term Disability & link to file. Answer questions on the injury and provide Dr information.
