Do I just say it straight, that the job is a mess, or do I clean it up so it sounds better? How honest am I supposed to be? If I say too much, does that sc--w me, and if I don’t, why am I leaving?

Do not bash your employer. Say you are ready for a new opportunity and that you feel the role you are interviewing for seems closely aligned to your career goals.
As to why you are leaving:
I’ve had a great experience where I am and have learned a lot, especially working with xyzyadayada. At this point, I feel ready for a new opportunity where I can continue to grow, take on new challenges, and contribute at a higher level.
Never ever trash your current employer, no matter what. Keep the real opinions to yourself and this site. I had to hold my tongue in a few interviews, but I still landed a case manager job after three weeks of searching and interviewing.
Just be honest - the company is struggling, many layoffs, restructuring. That you need employment, so you know you need to look since the risk is high. Then turn it back to you - what you did and the impact, what you can do, etc.
Hiring managers will understand!
