If yes, who decides? Your VP, SD or HRBP? I’m mentally exhausted and have about 6 months of savings. I know it’s a risky move but I think it’s the right thing to do to focus on my health.

Yes, but you need to verify that your manager or skip level will be laying off employees on their(your team) before you communicate your desire to exit. If you can’t verify this, you just seem weak.
I tried. Said I was ready to go months before the new structure was announced. Then was reorg’d into roll I didn’t want. Turned it down. No severance. Manager was cold as ice and no help. Had to fight on my own for a separation agreement which is only a % of severance. So be careful.
Be cautious, the job market isn't exactly inviting! Secure something first if you plan to continue working.
if you already have a job lined up it's a good strategy, you could try to time severance while also having a new job secured. Otherwise, tough it out till you can find work, never quit a good paying job due to "burn out".
That would be too human. Giving employees option to volunteer for restructuring takes away the power of those svp that know nothing about your job, but want to look like they do. And also consultants need to charge for their algorithms to pick people.

I would say start with your Senior Director and be blunt, mentioned that the past couple of years has taken some turmoil on your life and you wouldn't mind being let go in a reorganization if one came up. Sure, a bit awkward, but these leaders do talk to VPs when it comes to ranking best employees.
