Are we allowed to work outside of US being on US payroll systems? HR says no but I see so many IT folks working in India while being on US Payroll system.

There are special approvals that must be obtained in order to work in another country while on US payroll. Check WorkNet for specifics.
VPN notifies your manager if you try to login offshore.

I have asked to request 2 weeks outside of the country and it was denied by HR per my manager. Is it something they have to approve?
Some get exceptions due to any family emergency from few weeks to months. But it’s all depends on your manager.
2 weeks is allowed as per HR policy .
