Big corporations get complacent and things that seem to work aren't worried about. People change jobs, other things crop up that are funded and get attention and they never get back to completing that written procedure that becomes a quality specification you can train people too.

UAL's SFO MRO shop is FAR Part 121. They perform their own maintenance and have self-approved engine repairs (in some cases). It's all on them.
