How worried should Intel be about Microsoft developing own chips? How long would this take and is it even a reality?

Google was able to get their TPU silicon out and deployed in their data center in under 2 years.
Microsoft already have big hardware team in WA doing custom acceleration hardware for Bing and Azure, ready to dent Intel. Not a matter of if, it's when now.
Yes it will happen, yes it is a reality, and Microsoft is probably pretty far down the path to announce it. The impact will be small at first, but could be pretty significant over a couple of years especially if they decide to sell their design and components to other companies like Dell, HP, or Lenovo.
Intel upper management should be worried, but most of them will probably cash out options and jump ship before the train wreck happens.
So many applicants from Intel they just took about 1/3.
