Since they were previously with WIN I am confident this is trigger issue in the local switch. Not doing a LRN dip routing the call properly.

I just checked and they are ported to L3 haven't been with WIN since 2012.

Betting meadville PD used to have a windstream LRN, and triggers are set incorrectly in the switch.
