Who is deciding who is getting laid off and how? What are they basing their decisions on? Does HR have 100% control over this or are they considering input from managers and or senior folks?

If your management chain doesn't like you, then you are gone. It's universal.
HR has nothing to do with decision making, target %s come from the top and lower rank management need to hit the numbers - the manager decides on the name but he must follow allocations
Manager usually provides the name of the person to be laid off.
