This happens in every company.
Engineers bring forward issues to their management. Sometimes to the next level of management. However, one of two things happen.
- All their management hears is "this will be a schedule slip" they don't hear then issue or consequences.
- The front line managers bring it up the chain and the further it goes up the news is softened. As was said, promotions and bonuses are dependent upon schedule not correctness.
So, by the time it gets to mid to senior leadership the details have been lost and all that is understood is schedule and cost impact. That gets pushed back on because non-engineering management could not comprehend the technical issue anyway and engineers that make it up the chain are indoctrinated to "forget" what it was like to be an engineer.
This happens EVERYWHERE. The reason it gets attention at place like Boeing is because when your mocha machine does not work, it does not usually injure or k–l anyone.
So, both are true. Engineers brought it up AND upper management did not know. The problem is the layers of middle management that filter the news and the lack of technical ability at the higher levels.
