The medical department in my refinery requires us to filled out paperwork stating what illness we have and the treatment. How is that not a hippa violation?

@OP HIPAA doesn’t do what you think it does. It does more to protect doctors than anything else.
That said, most bosses wouldn’t be caught dead asking for this kind of information, because doing so can indirectly open them up to a lawsuit under certain circumstances. Keep in mind that nobody can “force” you to provide this kind of information. Just put “prefer not to disclose” and be done with it.
If someone tries to force you to sign anything under duress, decline politely and consult a licensed attorney. Do not take legal advice from the internet.
It's a benzene, lead, SO2, etc. exposure study by our 'medical' department. Cheaper than monitoring or PPE.

Covid made so many people think they understand HIPAA, and it’s just helped us identify the stupids that somehow survived the pandemic
Probably sign a paper saying you're willing to give them the information.....or not get paid
