Stop laying blame on supposed low-caliber hires. Ineffective management and outdated manual processes have to be key contributing factors for these errors to reoccur. Time to fire the useless layers of MDs since they're not solving these problems.

No, just the quality of people you get when you do not pay accordingly and when you lower the barrier to entry. Things are pretty bad especially in Risk - I have seen people who are so happy to be at Citi - they have been here for decades and do not plan to move because they know they can't get anywhere better - no one will hire them :D
Are these people making “near miss” Trojan Horses?
