Who determines the pricing on the co-pays, exams, etc.? Is it Charles Schwab or UHC?

I have always opted for the UHC Premier Copay plan as I have several medical issues. The 2026 plan has a pretty big increase in deductible, copays, and exams. I am just curious who determines these items should be raised, thanks in advance!

UHC and Kaiser. Schwab pays for a plan with the details worked out. Then Schwab funds said plan. The companies examine pool risk and history to tweak specifics of the plan. For example, if Schwab employees had more need for prescription A and didn’t use B, there might be a lowering of cost to benefit overall.
