How does management actually come up with their list of who they are laying off? Do mid-level managers have a say? Age? Salary? Performance? All the above? Asking for a friend.

Performance matters! If you are contributing to the bottom line, have a great attitude and can get along with people, you are a winner, wherever you work. Complainers and negative people are some of the first to go.
2016 a lot of the ones let go were over 50
The industry is still picking up. Don't think this is the end of the world and even if you survive, make sure you are prepared to do this over and over again for the rest of your career.
Disagree - leads have no input. Just hope your manager knows who you are and your supervisor is one that is willing to fight for you. If you rarely work with or talk to either one, you might as well start updating your resume.
It’s a personality contest. And yes leads have a say so.
