Background check? official HR responses to a background check are dates of employment, title, salary. the hard working people in India processing the background check won't know the particulars. most potential references will side with the ghoster and quietly support you
Who worries about it? Anyone who does that is too dumb to have a job.
If a company treated its employees right, they would not have to worry about it. When you have a management and HR team that is a failure, then you have a problem. I'm just saying!
