Ok, so we were handed a packet and was told to fill it out for DOT (Dept. Of Transportation) purposes. It's a an application for employment at Gregory and Howe Inc.! They're a d--g testing company out of Connecticut. I have no problem agreeing to random d--g screenings or whatever but why fill out an 'Application for Employment' with this company's logo at the top of every page. I shouldn't have to give my personal info, job history and driving record to a third party. Does anyone know what's going on here?

Union employees were not required to give info on this.
Okay point well taken ... What can be done about it from a Tech Level
The only vehicles that fall under DOT regulation are vehicles with a Gross Vehicle Weight Rating (GVWR) greater than 10,000 pounds.
They're about 100 Optimum trucks throughout the footprint that meet that weight. Most of them belonging to Construction/Fiber Department. The trucks are easily identifiable because they all will have a unique USDOT# sticker on it.
Any technicians operating these particular vehicles are required to have a valid DOT Medical Certificate. They're a small handful of Optimum vehicles that have a (GVWR) greater that 26,000 pounds. The technicians operating those vehicles are required to have a CDL and are subject to random d--g testing.
For many years, Management been mandating certain technicians to fill out suspicious DOT paperwork and have them get a suspicious DOT physical done. The problem in most cases, it never did not apply to them because the vehicles they operated were under 10,000 pounds.
For example.... OSP tech driving a Ford bucket truck should NOT be required to do anything related to DOT. Why? Because, the vehicle has a (GVWR) of 9,500 pounds.
Bottom line, If the truck you operate does NOT have a USDOT# sticker, DOT regulation does not apply to you whatsoever.
We have been doing that for years !! It’s for Dot to drive a truck over a Certain weight
