I know of situations that where similar to this, which i fought hard to fix and was successful, however not enough to prevent from getting terminated. At least i know i did the right thing, fighting for safety was my top priority when i was employed at Williams. Not saying that they dont care about safety but it wasnt a top priority with some, very sad, the CSB confirmed what i was seeing and telling people.

Poor weak management through and through. Not getting any better. People died because of this stupidity.

. Sometimes the SMEs have no teeth or don't know squat, hard to believe SMEs where not involved in this since it went unfixed for 12 years. The video is shocking, I would have cleaned this up immediately if assigned to do it.

I agree not bringing in SMEs on projects to save time, schedule and cost is a big safety problem and needs cleaned up! Seen it way too many times and people get rewarded for it or "terminated" for bringing in SMEs, big problem.

Tragic, but unfortunately not surprising. Before being laid off, as an SME I was supposed to be consulted on many MOCs, however, I rarely was.
