OP, there is no COBRA since Heald is out of business. You'll have to do the insurance exchange on healthcare.gov. It is tedious and time consuming. If you call the number on the website, they'll walk you through it.

All benefits terminate 4/30. You can rollover your 401K into another qualifying plan, but everything else goes away. If you have medical flex plan dollars, go crazy tomorrow. Load up on refillable prescriptions. Order and pay for new glasses. Get the kids to the clinic for immunizations and check-ups. Your flex plan dollars cease to exist after 4/30 as well.
