Has anyone left the firm before completing the signon and relocation bonus tenure ends? What happens ? Will they get behind employee to return the amount and if not will they send to collection or does Qualcomm usually not care at all?

Trust me they will get back whatever you agreed to give up if you leave early
I had a roommate that left before one year. I think relocation he kept but no sign on bonus. If I remember correctly you get to keep sign on if you stayed more than a year. If less, they will ask for a portion of it, not all of it depending how how many months you were at Q
I had the same concern since I wanted to bolt after just a few months. I basically stuck it out and learned to adapt and once I reached my 2 year anniversary I feel like I am stuck in the Qualcomm way. There is so little one individual can do to make a change due to the old guards in charge and with the intense amount of nepotism. I am keeping my job options open at all times. The 2 years will go by fast.
They have a form 0H-SH!7 you fill out. It goes like:
Reason for leaving, mark all that apply:
[x] I had no idea my manager was so scr3wed up.
[x] I had no idea my manager's manager was so scr3wed up.
[x] I had no idea HR was so scr3wed up.
[x] I had no idea everyone was so miserable and looking for an escape route.
And if you check all options, they'll say, "meh, keep the money. We know. We're sorry we didn't tell you sooner. Message me on Linked if you can get me a job. "
Why not run away when there are constant layoffs lol
