Since being let go I have all these forms to fill out - for health care - unemployment etc.
Do I use the Tampa address on my W2 the Headquarters at 388 or the address of the work location. I'm not sure65

I used the address on my W-2 form.
I think 388 if you are from NY office for unemployment
