Old airplane. Bad maintenance from airline known for bad maintenance. But combination of bad Boeing manufacturing, bad airline maintenance and bad FAA oversight will lead to more accidents.
That is the counter measure for door plugs liberating themselves from the fuselage. Activate emergency slides just in case of a mishap.
From reading the article it might just take us a few years to get all the bad workmanship behind us. Playing catch up is going to be a tuff one.
