Is it your manager or your manager's manager? H/r? How do they determine who gets what rating? I know a few who received 3s when I feel they should have received 2s. I know some who received 2s when I feel they should have received 3s.

Your rating is determined by your direct manager, but let's just say that his/her manager and HR both have major control over any ratings. I was asked by my manager to 'tweak' a few of my reviews at the suggestion of his global MD as HR thought they were too many 1 & 2's within his silo.
In my group, my manager fights with their manager over rankings.

Do the bare minimum until your time here is up. Since we're not being laid off based on merit, do yourself a favor and do the bare minimum until your time here is up. I think it's safe to say that the majority of us realize that Citi is not the best place to build your career, so there's no need to do anything but the bare minimum until you are laid off or leave on your own. Don't do the management any favors is my motto.

One thing I have noticed is even when I go above and beyond, my rating is still the same and my bonus doesn't change much. So why go the mile?

Your Manager does your ratings then they join a calibration session with their peers in your region/country depending how you operate. They then discuss the ratings and make adjustments based on these conversations.
Sometimes someone who may be a strong performer is actually not when compared to their peers. Its about how much impact you make overall and not just locally.
I always advise my team to think bigger. Join projects or initiatives that have a wide remit and other managers involved, that way when calibration happens you have more voices speaking about you.
Do not judge what your colleagues get because you truly have no idea what they do on a day to day to basis. You may think you know but you don't. Only your manager does. All you need to worry about is yourself and do the above and make sure your manager knows about it all year not just at review periods.
Don’t let a bad performance review cause you to have any self doubt. Performance reviews at Citibank are like a ro--h motel. Anything perceived as bad checks in and it doesn’t check out. You could be Jesus Christ, turning water into wine, and they’ll say, “I’m so sorry Jesus! I tried so hard to get you a good review, but it just didn’t quite work out.” Reviews are based on the bell curve and once you pi-s a key player off, right or wrong, they’ll throw a black marble table during your review to meet the curve.
By policy, your rating should be determined by your direct manager. But in reality, a proposition is submitted by your manager to the MD and the final rating is determined based on the MD's and HR's input - the input is based on years of service, previous ratings, other people in the team with similar ratings who might be eligible for promotion, etc. So yes, overall rating is determined by MD/HR at the end of the day. Line manager can support you/his rating proposal but that's about it. Example - let's say it is you and someone else who got great ratings this year and are eligible for promotion - if HR says that there is only one available spot for promotion this year, than things like years of service/brown-nosing by your line manager/etc. will come into play...
