Does anyone have an indicator of how the annual AVGO RSU refresh is determined?
1- Is it a percentage of base salary?
2- does it vest quarterly over 4 years?

No it's not related to your base salary, it's related to your professional grade. Each grade has a dollar value as guidance to be issued each year. Countries also have different "tiers" of allocations, US being the highest. These allocations can be affected by managers on an individual basis between 0% and 199% based on your performance. They vest quarterly over 4 years. Enjoy!

Just to clarify, do you mean every year you typically get 100-150% of your base in RSU's granted again? Or 100-150% is typically split over a 4 year period?
