@as this is an employee led effort by those already impacted. Read the description Anyone facing a retaliatory pip bp is already firing them. The form allows for basic details WITHOUT names and any email (anonymous burner welcome) can be used to protect identity and no name is required.

Is there a legal firm involved with this? If so, can you at least get them to be open about it?
Anyone who has experienced retaliation is going to feel wary about adding their details to an anonymous online form.
@aj unpopular opinion, people as d-mb as OP or anyone that fills this out likely deserve termination.
Yeah there is no way is anyone d-mb enough to fill out this form. It feels like P&C fishing for people to target.
