Was Landmark falsifying charts?

I had heard somewhere on one of these threads that LM falsified charts and created fake diagnosis. Is this true? My parents were Landmark members.

This is a broad question - there at one point was 15 providers in my territory with upwards of 100 in the whole state. It is possible a provider was doing one or both of these things but that could happen anywhere. It’s not Landmark specific.

That’s called Medicare fraud and needs to be reported. If you know about it and don’t report it you are just as guilty.

A patient called and complained that their provider never showed up for a visit, according to the patient's chart, the provider entered a note that he went to the patient's home and completed the visit. Nothing was ever done about it , they just scheduled another visit. So Yes.

I don't think most people around here should be referred to as "leaders" considering their corporate values only exist in their email signatures... but, to answer your question— yes.
