I have been having 50 hours week, nightmares, and sleep loss for past 3 months. I was about to apply for short term disability for 3 months. Now hit by CAP. What should I do at this stage? Can I still apply? Has anyone applied for it successfully?

Apply for STD. But be prepared to be let go once you return.
File for STD and FMLA so you are protected under FMLA. If you get approved, max out STD, LTD and FMLA. If applicable, max out paid caregiver leave - I think this is 2 weeks/80 hours paid and this is not counted against PTO. Know your state’s labor law too. You need to have plan in place before you return to work though. Prioritize and take care of yourself.
I did, successfully- like yours, it was real. They will wait for you to come back then make your life a living he-l. My suggestion is that you find a new job. I left last summer- I am still working on getting myself healthy. Good luck and be kind yourself. This place is a grain of sand.

Shouldn’t have any impact on whether or not you can file STD - that’s up to your doctor, not HR.
