Has anyone (or anyone you know) lost out on their severance? What happened? I’m not talking about not receiving it due to being offered another position. What are examples of things that could risk that?

If you take a job with UHG/Optum prior to your last day, with 90% of your base salary, no severance is paid. The termination is reversed and you are transferred to your new position. If you do not have a job (at Optum/UHC) by your last day and you sign your severance letter. You will be paid severance. If you are a rehire at Optum/UHC at a later date, you do not have to pay severance back.
If you get a job before the designated separation date and you will be making around 90% of current salary, then you will not sign the severance agreement. Because they will reemploy you before the elimination of the current position.

I would guess that being on any sort of corrective action would do it.
I don’t know but I’m going to take a guess and say if they can price you said anything negative to reflect back in the company or did something that broke policy they can use that to not pay you but I think this question would be better asked in a week or two since it takes a bit to get severance for those just laid off
