The culture of correcting mistakes

Last couple of years the management with their lost decisions and the illogical procedures created the culture of constantly correcting mistakes.
We had our share of challenges during the years but we always got things done fluidly and there was always the correct way to handle curtain tasks. Now three different managers say three different things. An employee does a certain task wrong and those same three managers have three different opinions on how to correct the mistake. So it takes ages to finish anything. When have we become so unorganized on so many levels?

I haven’t seen any mistakes from management.

Higher management never admitted their mistakes. The only cover sh-t up, or tell you that you're misinterpreted it or are wrong. Entire groups of market specialists like in SF and XM where ignored on their feedback.
See the Citrix products graveyard a perfect example of all the fuckups.

This boat with legacy technology vision, deserves to sink.
