Why their big plans often produce horrible results?

The problem is most executive hired are there to make a quick buck without any idea of ground realities.
Create a big hype about the plan in the end no results.
Bound their salaries and monetary benifits with their feedback/performance see how quickly things turn around.
But that's for people with self respect and dignity which I guess they cant even spell forget about having one

Uh, the "old guard" = Raghu. His big idea is to make our customers "consumption-obsessed" and "gee, everybody please buy SaaS and give up the cost-effective perpetual licenses you already own". No real understanding of the sales and operational efficiency needed for true transformation. This company has never had an effective COO.

It's a pejorative with no true meaning.

My observation is just a plain lack of courage to transform

That's just one of the factor or maybe major one
Look at the recent promotion
Just promoted for sitting and bootlicking
Seriously money is being wasted on these people
HR is only there to wrap up wrongdoing of the management and victimise the whistle blower
Hence more feedback on public portal than internal
