I can't fill out the form electronically and can't forward it to my personal email to print and scan. Were the rest of you able to fill it out online?

I used Nuance PDF to edit and sign the document electronically. They didn’t have any problems with it not being signed with ink.
Print, fill out, ink sign, scan pdf, email it back with your oxy email. They will reject you once you are done. Have fun.
Yes, you cannot forward that document. Print to PDF. Save PDF. Email that PDF to personal email.
The Q&A bullet #3 clearly indicates inked signature required. Print, fill in all 7 pages, sign, scan all 7 pages to PDF & attach to submission (from Oxy email only. )

Did you try to open it up in Adobe and use the “Fill & Sign” tool? That’s what I did. Just put the text boxes on each line you’re supposed to fill out.
