Is it HR, upper management, someone else? I can't imagine that middle management would have a say. Even some upper management has been let go. Who decides who stays and goes?

The people making the decisions on who goes are the same ones who got us into the situation we are in. Therefore, don't expect the cuts to make any sense whatsoever. Everyone is just out to save their own hide.
Upper management is making these decisions, with limited consultation from HR individuals at the top.
Middle management has absolutely NO say in the matter of who is let go, but are often left to face the employees who have thousands of questions.
